A Clinical Study of the Abdominal Hernia


Abstract
The abdominal hernia is a common abnormality in surgical field. There were inguinal hernia, incisional hernia, umbilical hernia, femoral hernia, hiatal hernia, diaphragmatic hernia, intestinal hernia.
We could made chart review of 790 cases among 950 cases of abdominal hernia which were operated on Dept. of Surgery, Han Yang University Hospital between 4th May 1972 and 30th April 1982(for 10 years).
The results were as follows;
1) The incidence of abdominal hernia was 8.4% of all cases operated in G.S.-Dept. during that period(950 cases out of 11,333 patients).
2) In the order of frequency, inguinal hernia were 741 cases(93.8%), incisional hernia 34 cases (4.3%), diaphragmatic hernia 6 cases (0. 76%), femoral hernia 4 cases (0.5%), internal hernia 3 cases(0.38%), umbilical hernia 2 cases(0.25%).
3) In the inguinal hernia;
(1) Male patients were 7.5 times as many as female patients. This show the definite predominance of male over female.
(2) Age incidence was 378 cases(51%; 1~5 years), 131 cases(17.7%; 6~10 years), 75 cases (10. 1%; 11~20 years), 58 cases(7.8%; 21~40 years), 60 cases(8. 1%; 41~60 years), 39 cases (5.3%; above 60 years). 68.7% of the cases was from at the birth up to 10 years of age.
(3) In the incidence of location; 460 cases(62.1%) were located in the right side. 233 cases (31.4%) were located in the left side and 48 cases(6.5%) were in the both side.
(4) Type of herniation; 704 cases(95%) were indirect form. 36 cases(4.9%) were direct form and only 1 case(0.1%) was both type. Indirect from was the largest number.
(5) Type of operation; Ferguson methods were 536 cases(72.4%). Bassini methods were 184 cases(24.8%). McVay methods were 14 cases(1.9%) and Halsted methods were 7 cases (0.9%). Ferguson methods was commonly applied to the infant and children. The other methods were applied to the adult.
(6) The reccurence rate was 1.2%.
4) In the incisional hernia, predominance of female over male(1.8 : 1) was shown. The most common prodromal disease of the incisional hernia in those cases was appendicitis.
5) The others;
Diaphragmatic hernias were 6 cases(0.8%). Among them, 3 cases were congenital and the others were traumatic hernia. There are commonly before the age of 10.
Internal hernias were 3 cases and they were herniated through the mesenteric defect. Femoral hernias were 4 cases and they were all female and right side.
Umbilical hernias were 2 cases.
